RESUMO

The urgent global health challenge posed by methicillin-resistant Staphylococcus aureus (MRSA) infections demands effective solutions. Antimicrobial peptides (AMPs) represent promising tools of research of new antibacterial agents and LyeTx I mn∆K, a short synthetic peptide based on the Lycosa erythrognatha spider venom, is a good representative. This study focused on analyzing the antimicrobial activities of LyeTx I mn∆K, including minimum inhibitory and bactericidal concentrations, synergy and resensitization assays, lysis activity, the effect on biofilm, and the bacterial death curve in MRSA. Additionally, its characterization was conducted through isothermal titration calorimetry, dynamic light scattering, calcein release, and finally, efficacy in a mice wound model. The peptide demonstrates remarkable efficacy against planktonic cells (MIC 8-16 µM) and biofilms (>30% of inhibition) of MRSA, and outperforms vancomycin in terms of rapid bactericidal action and anti-biofilm effects. The mechanism involves significant membrane damage. Interactions with bacterial model membranes, including those with lysylphosphatidylglycerol (LysylPOPG) modifications, highlight the versatility and selectivity of this compound. Also, the peptide has the ability to sensitize resistant bacteria to conventional antibiotics, showing potential for combinatory therapy. Furthermore, using an in vivo model, this study showed that a formulated gel containing the peptide proved superior to vancomycin in treating MRSA-induced wounds in mice. Together, the results highlight LyeTx I mnΔK as a promising prototype for the development of effective therapeutic strategies against superficial MRSA infections.

OBJECTIVES: To evaluate the anti-demineralizing effect of a mouthwash comprising pomegranate peel extract (PPE 3%), sodium trimetaphosphate (TMP 0.3%), and fluoride (F 225 ppm) in an in situ study, and to assess its irritation potential in an ex vivo study. METHODS: This double-blind crossover study was conducted in four phases with 7 days each. Twelve volunteers used palatal appliances containing enamel blocks, which were subjected to cariogenic challenges. The ETF formulation (PPE + TMP + F, pH 7.0), TF formulation (TMP + F, pH 7.0), deionized water (W, pH 7.0), and essential oil commercial mouthwash (CM, 220 ppm F, pH 4.3) were dropped onto the enamel twice daily. The percentage of surface hardness loss, integrated loss of subsurface hardness, calcium, phosphorus, and fluoride in enamel and biofilms were determined. In addition, alkali-soluble extracellular polysaccharide concentrations were analyzed in the biofilms. The irritation potential was evaluated using the hen's egg chorioallantoic membrane test through the vascular effect produced during 300-s of exposure. RESULTS: ETF was the most efficacious in preventing demineralization. It also showed the highest concentrations of calcium and phosphorus in the enamel and in the biofilm, as well as the lowest amount of extracellular polysaccharides in the biofilm. In the eggs, ETF produced light reddening, whereas CM led to hyperemia and hemorrhage. CONCLUSIONS: The addition of PPE to formulations containing TMP and F increased its anti-demineralizing property, and this formulation presented a lower irritation potential than the CM. CLINICAL RELEVANCE: ETF can be a promising alternative alcohol-free mouthwash in patients at high risk of caries.

OBJECTIVE: To evaluate the lifestyle and quality of life in Brazilian children and adolescents during the COVID-19 social distancing period in 2020. METHODS: This cross-sectional study evaluated children and adolescents (2-18 years of age) and their parents, who voluntarily participated in an online survey. Snowball sampling was used to recruit participants during the first 6 months of the pandemic. A questionnaire was used to characterize the study population. The PedsQL 4.0 and the EUROHIS-QOL 8-item index were used to assess the quality of life (QoL) in children/adolescents and parents, respectively. Data were analyzed using SPSS 18.0 statistical program through the ANOVA with post hoc Bonferroni analysis, student's t test, and the generalized estimating equation. RESULTS: Mean screen time increased from 2h pre-pandemic to 5h during the pandemic (p < 0.001), which was associated with a decline in PedSQL4.0 scores (from 75.7 ± 2.6 to 71.3 ± 13.7, p < 0.001). Unhealthy eating habits increased from 11% to 34% and were associated with worse QoL scores compared with improved or unchanged eating habits during the pandemic (69.7 ± 13.3 vs 72.80 ± 13.4 vs 76.4 ± 12.6; p < 0.001). Poor sleep quality increased from 9% to 31.7% and was associated with worse QoL scores compared to improved or unchanged sleep quality during the pandemic (67.3 ± 13.1 vs 74.5 ± 13.1 vs 76.8 ± 12.2; p < 0.05). Physical exercise was associated with better PedSQL4.0 scores (77.5 ± 12.3 vs 72.5 ± 14.4; p < 0.001). Children aged 2-4y old had the best QoLscores. CONCLUSIONS: Pandemic-related social distancing promoted significant lifestyle changes in children and adolescents, increasing screen time, reducing physical activity, and worsening food and sleep quality, which resulted in worse QoL scores.

We conducted a systematic review and meta-analysis to examine if individuals with reading difficulties (RD), mathematics difficulties (MD), or unspecified learning difficulties (ULD) experience internalizing problems (i.e., anxiety, depression, somatic complaints, and social withdrawal) to the same extent, and if the effect sizes are influenced by moderators (age, internalizing problems type, anxiety type, rater type, selection criteria, and attention control). We reviewed 2,806 studies published in English between January 2000 and April 2023. Our final sample consisted of 96 studies that reported effect sizes or data to calculate them (a total of 120 unique samples, 83,260 participants, age range = 7.3 - 34.8 years). Risk of bias and sensitivity were assessed. A random-effects model analysis revealed a significant and moderate overall effect size (Hedge's g = -.54), indicating that individuals with RD, MD or ULD experience more internalizing problems than their chronological-age (CA) controls. Follow-up analyses showed that neither learning difficulties type nor age of participants were significant moderators, but selection criteria (diagnosis versus screening) and internalizing problems type were. These findings suggest that individuals with RD do not differ from those with MD or ULD in internalizing problems, and all fare poorly compared to CA controls.

OBJECTIVE: to assess the effectiveness and safety of the peripherally inserted central catheter for hematopoietic stem cell transplantation. METHOD: this review will follow the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses, and the search steps will be presented through the flow diagram. The search strategy aims to locate both published and unpublished studies. No time or language restrictions will be applied. The review will consider experimental and observational studies that include adult and pediatric patients undergoing hematopoietic stem cell transplantation. Patients using peripherally inserted central catheters will be compared with those using other central catheters.

Os resultados de testes psicológicos são comumente usados para diagnosticar crianças que enfrentam dificuldades no processo de escolarização com algum transtorno/distúrbio, sem levar em conta as condições histórico-sociais, apresentando pouca possibilidade de superação da dificuldade no âmbito educativo. Por isso, pesquisadores têm proposto a realização de avaliações a partir da Psicologia Histórico-Cultural (PHC), teoria que se distingue teórica e metodologicamente das que embasam as avaliações tradicionais. Em busca de contribuir com esses estudos, o objetivo deste artigo é apresentar resultados de uma pesquisa acerca de procedimentos para a realização do processo de avaliação psicológica das queixas escolares. Foram planejadas e realizadas avaliações de oito crianças com tais queixas. Como resultado, apresenta-se uma síntese dos procedimentos realizados: encontro inicial com os familiares/responsáveis, os professores e as crianças, atividades com elas e encontro final para apresentar os resultados obtidos e possibilidades de intervenção. O procedimento considerou a formação social dos processos psicológicos a partir da análise da atividade-guia. Considera-se que este trabalho contribui para a prática do psicólogo que busca realizar avaliações psicológicas que se distinguem das tradicionais e para o desenvolvimento de posteriores pesquisas que englobem outros períodos do desenvolvimento e que tenham como finalidade mudanças no âmbito escolar, familiar e societário.


The results of psychological testing are commonly used to diagnose children who face difficulties in the schooling process with some disorder, without considering the historical and social conditions, bringing little possibility of overcoming the difficulty through the educational scope. For this reason, researchers have proposed assessments based on cultural-historical psychology (CHP), a theory methodologically different from those that underlie traditional assessments. Seeking to contribute to these studies, this paper aims to present the results of research that involved procedures for carrying out a psychological assessment of school complaints. Assessments of eight children with school complaints were planned and carried out. As a result, we present the following steps: initial meeting with family members/guardians, teachers, and the child, activities with children, and final meeting to present the results and possibilities for intervention. The whole procedure considered the social formation of psychological processes based on the analysis of the guide activity. We believe this work contributes to the practice of the psychologist who seeks to carry out psychological assessments that differ from the traditional ones and to the development of further research that involve other development periods and changes in the school, family, and society as a purpose.

This study investigated the anti-caries and anti-inflammatory effects of mouthwash formulations containing Punica granatum (pomegranate) peel extract (PPE), sodium-trimetaphosphate, and low concentrations of fluoride. PPE was characterized using high-performance liquid chromatography (ellagic acid and punicalagin). Total phenolics were quantified among formulations, and their stability was analyzed for 28 days. The formulation effects were evaluated as follows: (1) inorganic component concentration and reduced demineralization on bovine enamel blocks subjected to pH cycling; (2) anti-biofilm effect on dual-biofilms of Streptococcus mutans ATCC 25175 and Candida albicans ATCC 10231 treated for 1 and 10 min, respectively; and (3) cytotoxicity and production of inflammatory mediators (interleukin-6 and tumor necrosis factor-alpha). The formulation containing 3% PPE, 0.3% sodium-trimetaphosphate, and 225 ppm of fluoride resulted in a 34.5% surface hardness loss; a 13% (treated for 1 min) and 36% (treated for 10 min) biofilm reduction in S. mutans; a 26% (1 min) and 36% (10 min) biofilm reduction in C. albicans; absence of cytotoxicity; and anti-inflammatory activity confirmed by decreased interleukin-6 production in mouse macrophages. Thus, our results provide a promising prospect for the development of an alcohol-free commercial dental product with the health benefits of P. granatum that have been recognized for a millennium.

Hepatitis B viral infection (HBV) in prisons poses serious public health challenges because it significantly contributes to the increase in both morbidity and mortality indicators worldwide. Research has shown high HBV prevalence among inmates when compared to the general population. In this study, we estimated the prevalence of HBV exposure and its risk factors among 1,132 inmates detained in high security institutions. A cross-sectional, epidemiological study was carried out in 11 male-only prisons in the State of Paraná, Brazil, between May 2015 to December 2016. HBV exposure was explored using a variety of methods, including HBsAg, anti-HBs, and total anti-HBc. Data were analyzed using univariate and multivariate techniques. The overall prevalence of HBV exposure was 11.9% (95% CI: 10.9-12.8), totaling 135 individuals. In the multivariate analyses, risk factors that remained statistically significant were related to the penitentiary location (Francisco Beltrão; OR = 5.59; 95% CI: 3.32-9.42), age (over 30 years; OR = 5.78; 95% CI: 3.58-9.34), undergoing tattooing procedures in prison (OR = 1.64; 95% CI: 1.03-2.60), self-reported sexual activities with a known drug user (OR = 1.67; 95% CI: 1.12-2.48) and having a history of previous history of hepatitis B or C infection (OR = 2.62; 95% CI: 1.48-4.64). The findings indicate that public policies-including vaccination, early diagnosis, harm reduction strategies, and adequate treatment-should be designed and delivered in the same way for both the incarcerated and the general population in order to reduce the prevalence of HBV and its associated consequences.

OBJECTIVES: The aim of this study were to produce a multifunctional nanocomposite combining silver nanoaparticles (Ag), sodium trimetaphosphate (TMP) and fluoride (F), to investigate its effect on dental enamel demineralization and on biofilms of Streptococcus mutans and Candida albicans. METHODS: Bovine enamel blocks were submitted to five pH cycles and treated 2x/day with 100 ppm F, 225 ppm F, 100 ppm F + 0.2%TMP or 100 ppm F + 0.2%TMP+10% Ag (100F/TMP/Ag). Next, surface hardness loss (%SH), integrated loss of subsurface hardness (ΔKHN), enamel fluoride (F) and calcium (Ca) concentration were determined. Biofilms from single and dual species of S. mutans and C. albicans were treated with 100F/TMP/Ag, Ag or chlorhexidine gluconate for 24 h. The antibiofilm effect was evaluated by colony-forming unit counting and Scanning Electron Microscopy. RESULTS: The nanocomposite reduced 43.0% of %SH and was similar with samples treated with 225F, 100F/TMP and 100/TMP/Ag. The attribute of F and/or TMP in reducing ΔKHN in 5-20 µm was not affected by the addiction of Ag (110F = 225F = 100F/TMP = 100F/TMP/Ag > Negative Control). Further, 100F/TMP/Ag strongly reduced viable cells of S. mutans in dual biofilms (∼5 log10cm2) and structurally affected the biofilms. CONCLUSION: The 100F/TMP/F promoted a protective effect against enamel demineralization and was able to significantly inhibit the growth of biofilms of S. mutans and C. albicans. CLINICAL SIGNIFICANCE: The focus on prevention and non-invasive dental treatment is the most effective and least costly way to improve the population's oral health conditions. We present a nanocomposite for a multiple approach in prevention of caries.

O objetivo deste trabalho é descrever casos clínicos e séries de casos relacionados a alterações oftalmológicas em pacientes com diagnóstico de COVID-19. Foi realizada uma revisão narrativa/descritiva de casos clínicos e série de casos. A partir das buscas de dados com descritores pré-definidos, foram integrados na revisão, 17 estudos. Dentre os principais temas identificados, destacam-se: alterações conjuntivais, alterações retinianas e oftalmoparesias. O quadro de alterações da conjuntiva foi prevalente em relação aos demais. Essa revisão incluiu não apenas afecções oculares em adultos, mas também, em crianças e adolescentes. O estudo chama atenção para o fato de que as alterações oculares foram descritas como alteração isolada, alteração precipitante e alteração simultânea ao quadro respiratório. Conclui-se que as afecções oculares vão além de alterações conjuntivais, embora sejam essas preponderantes, havendo ainda alterações retinianas, quadro de oftalmoparesia e ainda a incomum síndrome de Miller Fisher. Novos ensaios irão poder avaliar, qual é de fato, a representatividade dos problemas oculares na cadeia epidemiológica da COVID-19. [au]


The objective of this work is to describe clinical cases and case series related to ophthalmological changes in patients diagnosed with COVID-19. A narrative/descriptive review of clinical cases and case series was performed. Based on data searches with pre-defined descriptors, 17 studies were integrated in the review. Among the main themes identified, the following stand out: conjunctival changes, retinal changes and ophthalmoparesis. The picture of changes in the conjunctiva was prevalent in relation to the others. This review included not only eye disorders in adults, but also in children and adolescents. The study draws attention to the fact that the ocular changes were described as isolated alteration, precipitating alteration and simultaneous alteration to the respiratory condition. It is concluded that ocular conditions go beyond conjunctival alterations, although these are predominant, with retinal alterations, ophthalmoparesis and the unusual Miller Fisher syndrome. New trials will be able to assess, in fact, the representativeness of eye problems in the epidemiological chain of COVID-19. [au]

Beyond the established difficulties of individuals with dyslexia in word recognition and spelling, it remains unclear how severe their difficulties in comprehension are. To examine this, we performed a meta-analytic review. A random-effects model analysis of data from 76 studies revealed a large deficit in reading comprehension in individuals with dyslexia compared to their chronological-age (CA) controls (g = 1.43) and a smaller one compared to their reading-level (RL) matched controls (g = 0.64). Individuals with dyslexia also differed significantly from their CA controls in listening comprehension (g = 0.43). Results further showed significant heterogeneity in the effect sizes that was partly explained by orthographic consistency (the deficits were larger in languages with low orthographic consistency) and vocabulary matching (the deficits were larger in studies in which the groups were not matched on vocabulary). These findings suggest, first, that individuals with dyslexia experience significant difficulties in both reading and listening comprehension, but the effect sizes are smaller than those reported in the literature for word reading and spelling. Second, our findings suggest that the deficits in reading comprehension are likely a combination of deficits in both decoding and oral language skills.

OBJECTIVE: to determine the prevalence of sleep quality and common mental disorder in Nursing professionals and factors associated with sleep change. METHOD: a cross-sectional, analytical and quantitative study developed with 196 Nursing professionals of a public hospital and a mixed one. Data was collected by means of an instrument of sociodemographic characterization, by the Self-Report Questionnaire 20 and Pittsburgh Sleep Quality Index and were analyzed by descriptive and inferential statistics to identify possible factors associated with sleep changes. RESULTS: sleep changes were identified among the Nursing professionals with a frequency of 76.5% (70.4-82.1). Sleep quality was classified as poor in 41.8% (41.8-55.6) and sleep disorder in 27.6%. (21.4-34.2). The prevalence of common mental disorder was identified in 36.7% (30.1-43.9). The main factor for poor sleep quality was the presence of common mental disorder (Odds Ratio: 5.15; p<0.001). CONCLUSION: sleep changes were prevalent and the characteristics of the work environment and the presence of mental disorder showed relevance in the changes.

ABSTRACT: Older adults with human immunodeficiency virus (HIV) have higher risks for early manifestations of age-related disabilities. The objective of this study was to compare HIV-positive and HIV-negative adults aged ≥50 years in relation to sociodemographic, behavioral, and geriatric characteristics. A case-control study was conducted with a >90% estimated statistical power. A total of 52 individuals living with HIV were matched by age, sex, and neighborhood of residence with 104 community controls. Age-related disabilities were assessed throughout a comprehensive geriatric assessment. Review of medical records and interviews were used to obtain behavioral and clinical covariates. No statistical differences on clinically significant age-related disabilities were found. However, multivariate regression analyses, controlling for education and income, revealed that behavioral (use of condom [odds ratio {OR}: 7.03; 95% confidence intervals {CI}: 2.80-7.65] and number of medical visits [OR: 1.25; 95%CI: 1.09-1.43]), along with faster gait speed (OR: 17.68; 95%CI: 2.55-122.85) and lower body and muscle mass indexes were independently associated with HIV (OR: .88; 95%CI: .79-.98 and OR: .72; 95%CI: .54-.97, respectively). In summary, results on age-related disabilities between groups could mean that public policies on HIV might be contributing to patients' positive outcomes regardless of the effects of aging, albeit gait speed, body and muscle mass indexes were independently associated with HIV. Screenings for age-related disabilities in specialized HIV services are recommended.

The purpose of this meta-analysis was to examine if individuals with dyslexia (DYS) have a deficit in orthographic knowledge. We reviewed a total of 68 studies published between January 1990 and December 2019, representing a total of 7215 participants. There were 80 independent samples in the chronological-age (CA)-DYS comparison and 33 independent samples in the comparison between DYS and reading-level (RL) controls. A random-effects model analysis revealed a large effect size (Cohen's d = 1.17) for the CA-DYS comparison and a small effect size (Cohen's d = 0.18) for the RL-DYS comparison. In addition, we found significant heterogeneity in the effect sizes that was partly explained by the level of orthographic knowledge (effect sizes being higher for lexical than sub-lexical orthographic knowledge). These results suggest that individuals with dyslexia experience an orthographic knowledge deficit that is as large as that of phonological awareness and rapid automatized naming reported in previous meta-analyses.

Objective: to determine the prevalence of sleep quality and common mental disorder in Nursing professionals and factors associated with sleep change. Method: a cross-sectional, analytical and quantitative study developed with 196 Nursing professionals of a public hospital and a mixed one. Data was collected by means of an instrument of sociodemographic characterization, by the Self-Report Questionnaire 20 and Pittsburgh Sleep Quality Index and were analyzed by descriptive and inferential statistics to identify possible factors associated with sleep changes. Results: sleep changes were identified among the Nursing professionals with a frequency of 76.5% (70.4-82.1). Sleep quality was classified as poor in 41.8% (41.8-55.6) and sleep disorder in 27.6%. (21.4-34.2). The prevalence of common mental disorder was identified in 36.7% (30.1-43.9). The main factor for poor sleep quality was the presence of common mental disorder (Odds Ratio: 5.15; p<0.001). Conclusion: sleep changes were prevalent and the characteristics of the work environment and the presence of mental disorder showed relevance in the changes.


Objetivo: determinar a prevalência da qualidade do sono e do transtorno mental comum em profissionais de enfermagem e os fatores associados à alteração do sono. Método: estudo transversal, analítico e quantitativo desenvolvido com 196 profissionais de enfermagem de um hospital público e um misto. Os dados foram coletados por meio de instrumento de caracterização sociodemográfica, do Self-Report Questionnaire 20 e do Índice de Qualidade de Sono de Pittsburgh e foram analisados por estatística descritiva e inferencial para identificar possíveis fatores associados com a alteração no sono. Resultados: a alteração do sono foi identificada entre os profissionais de enfermagem numa frequência de 76,5% (70,4-82,1). A qualidade do sono foi classificada como ruim em 41,8% (41,8-55,6) e o distúrbio do sono em 27,6% (21,4-34,2). Identificou-se uma prevalência de transtorno mental comum em 36,7% (30,1-43,9). O principal fator para a má qualidade do sono foi a presença de transtorno mental comum (Odds ratio: 5,15 p<0,001). Conclusão: as alterações de sono foram prevalentes e as características do ambiente de trabalho e a presença de transtorno mental comum apresentaram relevância com as alterações.

Método: se trata de un estudio transversal, analítico y cuantitativo desarrollado con 196 profesionales de enfermería de un hospital público y otro mixto. Los datos se recolectaron mediante un instrumento de caracterización sociodemográfica, utilizando el Self-Report Questionnaire 20 y el Índice de Calidad de Sueño de Pittsburgh y se analizaron utilizando estadísticas descriptivas e inferenciales para identificar posibles factores asociados con trastornos del sueño. Resultados: se identificaron trastornos del sueño entre los profesionales de enfermería con una frecuencia del 76,5% (70,4-82,1). La calidad del sueño se clasificó como mala en el 41,8% (41,8-55,6) y trastorno del sueño en el 27,6% (21,434,2). Se identificó una prevalencia de trastorno mental común en el 36,7% (30,1-43,9). El factor principal de la mala calidad del sueño fue la presencia de trastorno mental común (Odds ratio: 5,15 p<0,001). Conclusión: los trastornos del sueño fueron prevalentes y las características del entorno laboral y la presencia de trastorno mental común fueron relevantes para los trastornos del sueño.

The paper presents an exploratory and descriptive quantitative study, involving 560 Portuguese parents with school children, aged between 6 and 17 years. Data collection was carried out through a questionnaire that was made available online and focused on the use of social networks, the Internet and the risk of online grooming. The results showed that about half of the participants did not know the term 'online grooming'. After defining the phenomenon, 97% of the participants considered it very seriously. To prevent risk, 52% of the participants reported talking with their children about online risks and taking precautions to reduce the children's vulnerability. Approximately 89% of the respondents said they knew where to report cybercrime. The study aimed to contribute to a greater awareness of the risks associated with the Internet, involving parents and caregivers in the prevention of situations of victimization of children and adolescents. O artigo apresenta um estudo quantitativo exploratório e descritivo, envolvendo 560 pais portugueses que tinham filhos em idade escolar entre 6 e 17 anos. A recolha de dados foi feita por meio de um inquérito por questionário, disponibilizado online, focado no uso das redes, da internet e no risco do online grooming. Os resultados mostraram que cerca de metade dos participantes não conhecia o termo "online grooming". Depois de definir o fenómeno, cerca de 97% dos participantes consideram-no muito grave. Para prevenir, 52% dos participantes disseram conversar com seus filhos sobre os riscos online e tomar precauções para reduzir a vulnerabilidade das crianças. Cerca de 89% dos entrevistados disseram saber onde denunciar crimes cibernéticos. O estudo visa contribuir para uma maior conscientização dos riscos associados à internet, envolvendo pais e cuidadores na prevenção de situações de vitimação de crianças e adolescentes. (AU)


El artículo presenta un estudio cuantitativo exploratorio y descriptivo, en el que participaron 560 padres portugueses que tenían hijos en edad escolar entre 6 y 17 años. La recolección de datos se llevó a cabo a través de una encuesta, disponible en línea, enfocada en el uso de redes, Internet y el riesgo del online grooming. Los resultados mostraron que aproximadamente la mitad de los participantes no conocían el término "online grooming". Tras definir el fenómeno, alrededor del 97% de los participantes lo consideró muy grave. Para prevenirlo, el 52% de los encuestados afirmaron hablar con sus hijos sobre los riesgos digitales y tomar precauciones para reducir la vulnerabilidad de los niños. Alrededor del 89% de los encuestados dijeron que sabían dónde denunciar los delitos cibernéticos. El estudio tiene como objetivo contribuir a una mayor conciencia de los riesgos asociados a Internet, involucrando a los padres y cuidadores en la prevención de situaciones de victimización infantil y adolescente. ABSTRACT Objective The present study assessed the differences in nutritional markers (albumin, transferrin, total body fat, and body mass index) and clinical complications (diarrhea, vomiting, and diet interruptions) associated with enteral nutrition. Methods This is an open-label, randomized, two-arm parallel-group controlled clinical trial. Out of 105 patients assessed for eligibility, 35 adult patients were randomly divided into two groups and followed for 28 days. The sample comprised a heterogeneous group of severely ill individuals initially treated in the intensive care units. Neurological conditions (i.e., strokes and brain tumours) were the most common reasons for hospitalization. Patients had one singularity: the clinical need for exclusive enteral nutrition therapy. One group received the diet via gastric tube and the other via a post-pyloric tube. Results The groups presented increases in the calories prescribed and administered, as well as reduced diet discontinuation. Although similar values were observed up to day 21, the post-pyloric group showed increased albumin levels compared to the gastric group on Day 28. Transferrin levels increased over time in both groups. Conclusion There were no differences in the complications recorded between groups, albeit serum albumin significantly increased in the post-pyloric group.


RESUMO Objetivo Avaliar diferenças tanto nos marcadores nutricionais (albumina, transferrina, gordura corporal e índice de massa corporal) quanto nas complicações clínicas (diarreia, vômitos e interrupções na dieta) associadas à nutrição enteral administrada através de duas vias tradicionais. Métodos Este é um ensaio clínico de tratamento, paralelo de dois braços, aberto e randomizado controlado. Dos 105 participantes avaliados para elegibilidade, 35 pacientes adultos foram divididos aleatoriamente em dois grupos e seguidos por 28 dias. A amostra foi formada por um grupo heterogêneo e gravemente enfermo, tratados inicialmente em unidades de terapia intensiva. Condições neurológicas, como acidente vascular e tumores cerebrais foram as razões principais para hospitalização. Os pacientes tinham em comum um aspecto, a saber, a necessidade clínica exclusiva de receber nutrição enteral. Um grupo recebeu a dieta via sonda gástrica e o outro através de sonda pós-pilórica. Resultados Os grupos apresentaram aumento de calorias prescritas e administradas, bem como redução da descontinuação da dieta. Embora valores semelhantes tenham sido observados até o 21° dia, o grupo pós-pilórico apresentou aumento dos níveis de albumina em relação ao grupo gástrico no dia 28. Os níveis de transferrina aumentaram ao longo do tempo em ambos os grupos. Conclusão Não houve diferenças nas complicações registradas entre os grupos, embora níveis séricos de albumina aumentaram significativamente no grupo pós-pilórico.
